Gordon Memorial Hospital District offers Diabetes Self-Management Education and Support (DSMES) as a personalized education program. The DSMES program helps individuals with diabetes understand their condition and manage their health with confidence.
Diabetic Education Services
Our diabetic education program provides individual education focused on the knowledge and skills needed to manage diabetes day to day.
This program includes:
- One individual admission appointment
- Four individual education sessions, approximately 1.5 hours each
- Follow-up support available as needed

Education will focus on eight key areas of diabetes self-management:
- Understanding Diabetes – Learn how diabetes affects your body and how it can be managed.
- Healthy Eating – Understand how food and nutrition affect blood glucose and learn practical strategies for healthy eating.
- Physical Activity – Learn how physical activity can affect diabetes and ways to incorporate activity into your routine.
- Medication Management – Better understand your diabetes medications and how they fit into your treatment plan.
- Monitoring – Learn about blood glucose monitoring and how to understand your results.
- Problem Solving – Develop strategies for handling challenges that may arise while managing diabetes.
- Reducing Risks – Learn ways to reduce the risk of diabetes-related complications.
