Community Health Improvement Plan
At Gordon Memorial Hospital District, we are proud to be part of a community-wide effort to support the health and well-being of the people we serve. One important way we do this is through the Community Health Improvement Plan, or CHIP.
We work closely with Panhandle Public Health District and other local partners to identify the most important health needs in our region. These needs are based on health data, community feedback, and ongoing assessments.
The CHIP outlines goals and strategies that focus on real issues affecting our area. This includes improving access to care, supporting mental health, preventing chronic disease, and encouraging healthy living.

Help Shape the Future of Healthcare in Our Community
Panhandle Public Health District is inviting community members to participate in an important community health survey. The goal is to collect 2,000 responses to help guide upcoming prioritization planning meetings and future healthcare initiatives for the 2027-2029 Community Health Improvement Plan (CHIP).
This survey takes approximately 15 minutes to complete and is completely anonymous.
Questions focus on key issues that impact daily life in our community, including:
- Affordable housing
- Access to healthcare services
- Food availability
- Childcare access
- Other community health needs
